9) Insurance Credit Score Notice

Many insurance carriers use credit information to generate an “insurance credit score,” which may be based on factors such as public records, payment history, length of credit history, credit inquiries, number of open accounts, types of credit, and outstanding debt.

Information collected (such as name, address, and Social Security number, when provided) may be used by insurance carriers to obtain an insurance credit score.
